Medical Director, Child and Adolescent Eating Disorder Programs

Dr. Tracey Cornella-CarlsonTracey Cornella-Carlson, MD, a board-certified child, adolescent and adult psychiatrist, is the medical director of the child and adolescent inpatient and the partial hospitalization eating disorder programs at Rogers Memorial Hospital – Milwaukee. She received her medical degree from the Chicago Medical School in 1992. At Rush Presbyterian St. Luke's Medical Center in Chicago, Dr. Cornella-Carlson completed a three-year general psychiatry residency and a two-year fellowship in child and adolescent psychiatry, during which she was Chief Fellow. She has done research on co-morbidity in boys with Attention Deficit/Hyperactivity Disorder, and specializes in eating disorders.

Dr. Cornella-Carlson is licensed to practice psychiatry in the state of Wisconsin, and a current member of the Academy for Eating Disorders and the American Academy of Child and Adolescent Psychiatry.

Dr. Stephanie EkenStephanie C. Eken, MD, a child and adolescent psychiatrist, earned her medical degree from the University of Tennessee College of Medicine in 2002 and her bachelor's degree from the University of Richmond. She served as chief resident during her triple board residency training in pediatrics, psychiatry, and child and adolescent psychiatry at the University of Kentucky.


Eken received the "Catcher in the Rye Award for Advocacy" from the American Academy of Child and Adolescent Psychiatry in 2008 and has received numerous other honors and fellowships. She has both authored and presented to professional and academic audiences. Her clinical interests include depression, anxiety, obsessive-compulsive disorder, attention deficit hyperactivity disorder, psychotic disorders, and emotional problems in chronically medically ill children.

Dr. Stephanie EkenJerry L. Halverson, MD, is a board-certified adult psychiatrist with a subspecialty in psychosomatic medicine. He serves as medical director for adult services at Rogers Memorial Hospital - Oconomowoc.

Dr. Halverson has been honored by his colleagues for clinical and leadership excellence. He has been selected for a 2010 "Forward under Forty" award by the University of Wisconsin Alumni Association and has also been awarded an American Medical Association Foundation 2010 leadership award for outstanding community and peer leadership as an early career physician.

Dr. Halverson is a recognized leader within the Wisconsin Medical Society and the Wisconsin Psychiatric Association and has held key leadership positions within professional organizations both locally and nationally. He has spoken extensively on mental health disorders and advocacy efforts to academic, legislative and community audiences.

Prior to joining Rogers Memorial, Dr. Halverson was the medical director of adult psychiatry at Meriter Hospital in Madison as was an assistant professor of psychiatry at the University of Wisconsin School of Medicine and Public Health where he directed the Treatment Refractory Psychiatric Disorders Program.

Dr. Jay KasnerJay Kasner, MD is the medical director of Chemical Dependency Services at Rogers Memorial Hospital – Milwaukee and the attending physician at The Herrington Recovery Center. Dr. Kasner is board certified as an addictionologist by the American Society of Addiction Medicine (ASAM). He is respected throughout the medical community for his clinical expertise with inpatient, residential and outpatient treatment programs.

Prior to joining Rogers Memorial, Dr. Kasner served as director of chemical dependency services at Charter Behavioral Health in Milwaukee and as medical director of Kettle Moraine Hospital in Oconomowoc. For more than 25 years, Dr. Kasner has limited his medical practice to treating people who are chemically dependent. This focus gives him a unique understanding of the pressures many of our patients face as they learn how to overcome the disease of addiction and live more productive lives.

Dr. Vineet KulkarniDr. Kulkarni, the medical director of Adult Services of Rogers Memorial Hospital–Milwaukee, is a board certified psychiatrist, and specializes in the treatment of adults and adolescents. He specializes in the treatment of attention deficit hyperactivity disorder (ADHD), affective disorders and anxiety disorders. Dr. Kulkarni received his MBBS (our equivalent of M.D.) at T.N. Medical College-Bombay India and completed residency at the University of Illinois Medical College of Psychiatry.

Dr. Peter LakeDr. Lake is a board-certified child, adolescent, and adult psychiatrist and is the medical director of Rogers Memorial Hospital-Oconomowoc, Child and Adolescent Services-Oconomowoc and the Child & Adolescent Center.

Dr. Lake has been in private practice since 1991, serving as vice chairman in the Department of Psychiatry at Physicians Plus Medical Group, S.C.; and associate medical director for Physicians Plus Mental Health Case Management and Consultation System, Madison until fall of 1999. He was also an associate clinical professor at the University of Wisconsin Medical School. Currently, Dr. Lake practices full time at Rogers Memorial.

Dr. Lake's clinical area of interest includes early childhood development, attachment disorders, attention deficit hyperactivity disorder (ADHD), childhood depression and anxiety disorders, medication management, and individual psychotherapy and family therapy. He is a frequent lecturer and consultant to schools, social service agencies, and case management systems. Additionally, Dr. Lake is a co-founder of Challenge Camp and a board member of the Rogers Memorial Hospital Foundation, a charitable organization for patients needing financial assistance to complete treatment.

Dr. Lake received his medical degree from the University of Iowa. He completed his residency in adult psychiatry and fellowship in child and adolescent psychiatry at Vanderbilt University in Nashville, Tennessee. He was the recipient of the 1989 Marc C. Hollender M.D. Award for Outstanding Performance by a Psychiatric Resident, Vanderbilt University, Department of Psychiatry; and 1982 Superior Academic Achievement Award, Creighton University in Omaha, Nebraska, where he completed his undergraduate degree.

Dr. Tim LevenhagenDr. Levenhagen is board certified in adult psychiatry and a full-time attending psychiatrist with the acute adult inpatient unit at Rogers Memorial Hospital – Oconomowoc. Mood and anxiety disorders, as well as the geriatric population are his major areas of clinical interest.

Dr. Levenhagen received his undergraduate and medical degrees from the University of Wisconsin – Madison. While completing his residency program in psychiatry at the Medical College of Wisconsin, Dr. Levenhagen was the Chief Psychiatric Resident at the VA Hospital in Milwaukee, Wisconsin.

Dr. Roger LuhnDr. Luhn is a board-certified child and adolescent psychiatrist and a full time medical staff member of the Child & Adolescent Center at Rogers Memorial Hospital in Oconomowoc. Prior to joining Rogers Memorial, Dr. Luhn was a medical director at the Mercy Cooperative Childcare Institute in Janesville, and a Mendota Mental Health Institute child and adolescent psychiatrist in Madison. He has directed inpatient, outpatient and residential programs for children and adolescents.
Dr. Luhn earned an undergraduate degree in biology at Whitworth College in Spokane, Washington. He earned his medical degree at the University of California in San Diego, California. Dr. Luhn is a member of the American Academy of Child and Adolescent Psychiatry.

Dr. Carol MullenDr. Mullen is a full-time psychiatrist and addictionologist in adult services at Rogers Memorial Hospital-Milwaukee. Previously, she was a staff psychiatrist at St. Michael's Hospital in Milwaukee. Dr. Mullen began her medical career as a registered nurse, and then completed a three-year internal medicine residency and a three-year psychiatric residency at Sinai Samaritan Medical Center in Milwaukee. She is certified in addiction medicine through the American Society of Addiction Medicine and is experienced in treating patients dealing with dual diagnosis as well as general adult psychiatric concerns.

Dr. Jody PahlavanDr. Pahlavan is a clinical psychologist licensed by the State of Wisconsin and is the program director of the Child & Adolescent Services Day Treatment Program of Rogers Memorial Hospital–Milwaukee. Dr. Pahlavan has been in private practice since 1995 and has more than 15 years of experience working with children and adolescents.

Dr. Pahlavan received her Doctor of Psychology degree from the Wisconsin School of Professional Psychology. She completed her residency in child psychology at Children’s Hospital of Wisconsin. She completed her master’s degree in psychology from Marquette University and her bachelor’s degree in special education from Cardinal Stritch College.

Dr. Pahlavan has given numerous talks on behavioral health issues affecting youth. She is a member of the American Play Therapy Association, the American Psychological Association, the Wisconsin Psychological Association, Children and Adults with Attention-Deficit/Hyperactivity Disorder (CHADD), and is listed with the National Register of Health Care Providers.

Dr. Kambiz PahlavanDr. Pahlavan is the medical director of the Rogers Memorial Hospital - Milwaukee and Child & Adolescent Services of Milwaukee. His primary objective is to continue Rogers Memorial's tradition of excellence in identifying the mental health needs of the community and developing high quality services within the hospital, as well as promoting education and research.

Dr. Pahlavan has worked at McLean Hospital, a teaching hospital of Harvard Medical School, where he clinically supervised child psychiatry fellows and general psychiatry residents, and presented weekly seminars on child psychiatry. Since 1993, Dr. Pahlavan worked with the Medical College of Wisconsin as the clinical supervisor for medical students and several general psychiatry residents. With more than 25 years of psychiatry experience, he has developed numerous lectures including adolescent suicide, Attention Deficit Hyperactivity Disorder (ADHD), pervasive developmental disabilities, autism (including spectrum disorders) and child and adolescent affective disorders. He has presented papers at the American Psychiatric Association and the AACAP. He was the 1999 recipient of the American Psychiatric Association’s Nancy C.A. Roeske Certificate of Excellence in Medical Student Teaching.
Dr. Pahlavan is board certified through the American Board of Psychiatry and Neurology and the American Board of Child Psychiatry. He completed his general psychiatry residencies at Norwich Hospital, Connecticut; Medfield State Hospital, Massachusetts; and New England Medical Center in Boston. He completed his fellowship in child psychiatry at the McLean Hospital, Harvard Medical School in Massachusetts. He is a fellow of American Psychiatric Association and the American Academy of Child & Adolescent Psychiatry.

Clinical Director, The Obsessive-Compulsive Disorder Center and Cognitive-Behavioral Therapy Services

Dr. Brad RiemannDr. Riemann is a leading expert in the assessment and cognitive-behavioral treatment of anxiety disorders and supervises the training program for graduate and post-graduate students from around the country for cognitive behavioral therapy in anxiety disorders.

Dr. Riemann is on the Scientific Advisory Board of the Obsessive-Compulsive Foundation and the Obsessive-Compulsive Foundation of Metropolitan Chicago as well as a member of the Publications Committee of the Anxiety Disorders Association of America. He is past president of the Board of Directors of the Mental Health Association in Waukesha County.

He has authored numerous scientific papers on obsessive-compulsive disorder and anxiety and has spoken at national and international conventions, including the Association for the Advancement for Behavioral Therapy, the American Anxiety Disorder Association and the Obsessive-Compulsive Foundation.

He collaborates with colleges and universities all over the country on research projects investigating obsessive-compulsive disorder and anxiety. Dr. Riemann has also been featured on the national television shows 48 Hours and The Today Show.

Dr. Riemann received his doctorate in clinical psychology from the Chicago Medical School. He is also a clinical assistant professor in the department of psychology at the Chicago Medical School, Marquette University, and the University of Wisconsin-Milwaukee.

Ulland, MDErik Johnston Ulland is a 2003 graduate of Loma Linda University School of Medicine and performed residency at the University of Wisconsin Hospital and Clinic's Child Psychiatry program from 2006 to 2008, and at Dartmouth-Hitchcock Medical Center in Lebanon, N.H. from 2003 to 2006.

 

 

 

Medical Director, Eating Disorder Services

Dr. Ted WeltzinTheodore E. Weltzin, MD is a board-certified adult psychiatrist who is an established leader in the field of eating disorders.

He serves on the board of directors for the Eating Disorder Coalition, is a member of the American Psychiatric Association and Academy of Eating Disorders and an Assistant Clinical Professor of Psychiatry at the Medical College of Wisconsin.

Dr. Weltzin has contributed to the research and treatment of eating disorders with more than 45 peer-reviewed publications, invited book chapters and international lectures. He was a reviewer and consultant for the first American Psychiatric Association “Practice Guidelines for Eating Disorders” published in 1993 and is an Editorial Board Member for the journal “Eating Disorders: The Journal of Treatment & Prevention.”

Dr. Weltzin has appeared as an expert on CNN, CBS Morning News, Good Morning America, The View and The Dr. Phil Show.

As medical director of Eating Disorder Services at Rogers Memorial Hospital since 2001, Dr. Weltzin has provided leadership in all areas of development and clinical oversight of one of the largest eating disorder treatment centers in the country.

He has trained well over 100 clinicians in the field and developed innovative programs addressing the unique needs of eating disorder patients. These programs include residential, inpatient and partial hospital treatment as well as programs for patients with eating disorders and anxiety, excessive exercise and males with eating disorders.

Dr. Azhar M. YunusAzhar M. Yunus, M.D., is a full-time physician for Child and Adolescent services at our Milwaukee hospital. Yunus is a 1999 graduate of Saba University of School of Medicine in the Netherlands, and performed his residency at the University of Wisconsin Hospital and Clinics from 2004 to 2008.
